EFTPOS & HICAPS Facilities are available for your convenience. EFTPOS & HICAPS receipts will be issued. We do not accept Diners or American Express Cards. We email Receipts. Please confirm your email address with our Reception Staff. Please feel free to ask any questions.

RESEARCH

We are currently conducting research into the effectiveness of a 6 week course of physiotherapy treatment on conditions affecting the neck and jaw. At your initial assessment, our Administration team may ask you to sign a consent form and complete two surveys regarding your pain. Following your appointment, once you have been assessed, your Physiotherapist will then ask you to complete another 1 to 2 surveys depending on your condition (these will be emailed to you). 6 weeks following your initial assessment, we will re-email you the surveys to gauge the difference after a treatment course. There is also an option to review at 3 & 6 months. These surveys all take less than 2 minutes each to complete and this research is the first of its kind in the world.

Please note, you do not have to be a part of this research and can also opt out at any point. Please let the Administration staff know if you do not wish to participate. However, your interest in being a part of the study is greatly appreciated by us and goes a long way in helping our clinic reach our goal of generating a publishable paper detailing the change in pain levels and range of motion after a 6 week treatment period. PRIVATE HEALTH INSURANCE

If you have private health insurance please bring your card along and we can process your claim on the spot and you will only need to pay the gap payment. For details on your health fund rebate, please contact your health fund. The Item Number for an Assessment Consultation is #500 & Item Number #505 for a Subsequent Consultation. 

MEDICARE

If you have been referred to our practice by your GP under the Medicare Chronic Disease Management Program please inform our Reception Staff in order that the necessary paperwork can be processed. The Headache, Neck & Jaw Clinic does not have facilities to claim directly through Medicare. We do not bulk bill. We ask you pay your account on the day, and then make your claim through Medicare.

WORK COVER

You must provide a Worker Cover Medical Certificate to commence your treatment. The Headache, Neck & Jaw Clinic requests that patients pay on the day of their treatment and claim directly from Work Cover. Our Reception staff will provide you with any information.

COMPULSORY THIRD PARTY CLAIMS (CTP)

You must provide a Medical Certificate to commence your treatment. The Headache, Neck & Jaw Clinic requests that patients pay on the day of their treatment and claim directly from the insurer. Our Reception staff will provide you with any information.

DEPARTMENT OF VETERANS AFFAIRS (DVA)

If you wish to be treated under the DVA Scheme you can book appointments between 10.30am and 2.00pm with any of our Associate Physiotherapists at either our Nundah or Coorparoo Clinic. Treatment by Scott Cook or Nigel Smith will require a full consultation fee to be paid by the patient. If you have been referred to our practice by your GP under the DVA scheme you must provide a D904 referral from your Doctor and show your DVA Card in order that the necessary paperwork can be completed. Please see our Reception staff for a full copy of our policy.

SMS REMINDERS

As a courtesy we send an SMS reminder the day before your appointment. This is a reminder, not a diary note. We ask that patients take responsibility for remembering appointments. DO NOT REPLY TO OUR SMS, WE DO NOT RECEIVE REPLIES. If you need to change your appointment, we ask that you please call our office so that we can organise this for you. 

CANCELLATION POLICY

If you are unable to keep your appointment we require 24 hours notice.  Please call the day before or leave a message on our answering machine before 7.00am on the day. Failure to provide notice will attract a $60 cancellation fee.
