EFTPOS & HICAPS Facilities are available for your convenience. EFTPOS & HICAPS receipts will be issued. We do not accept Diners or American Express Cards. We email Receipts. Please confirm your email address with our Reception Staff. Please feel free to ask any questions.

PRIVATE HEALTH INSURANCE

If you have private health insurance please bring your card along and we can process your claim on the spot and you will only need to pay the gap payment. For details on your health fund rebate, please contact your health fund. The Item Number for an Assessment Consultation is #500 & Item Number #505 for a Subsequent Consultation. 

MEDICARE

If you have been referred to our practice by your GP under the Medicare Chronic Disease Management Program, please inform our Reception Staff in order that the necessary paperwork can be processed.

At The Headache, Neck & Jaw Clinic, we have the facilities to process the claim for you (provided you are eligible*). You will need to pay for the full fee up front and we can then process your claim and the money will go back into your bank account. If you are not eligible, we ask that you pay our normal fee up front and we then provide you with a receipt, which you can take to Medicare to claim your rebate or upload the receipt to an app on your smart phone and they will transfer the money back into your account.

* To be eligible to claim the rebate in one of our clinics, there are a few things we need to make you aware of. Firstly, if the paperwork has not yet been submitted by your GP or processed by Medicare, your claim will be denied and this will mean that you will need to claim at a Medicare office or through the smart phone app. Secondly, for our team to process a rebate, you will need to have a Savings or Cheque account card. Medicare do not supply rebates to Credit Cards. Lastly, if you would like us to process your claim for you, please allow adequate time for processing as these claims can take up to 3 minutes. The Headache, Neck & Jaw Clinic reserve the right to decline processing a claim if time does not permit.

WORK COVER

You must provide a Worker Cover Medical Certificate to commence your treatment. The Headache, Neck & Jaw Clinic requests that patients pay on the day of their treatment and claim directly from Work Cover. Our Reception staff will provide you with any information.

COMPULSORY THIRD PARTY CLAIMS (CTP)

You must provide a Medical Certificate to commence your treatment. The Headache, Neck & Jaw Clinic requests that patients pay on the day of their treatment and claim directly from the insurer. Our Reception staff will provide you with any information.

DEPARTMENT OF VETERANS AFFAIRS (DVA)

If you wish to be treated under the DVA Scheme you can book appointments between 10.00am and 2.00pm with either Jessica Dowling, Ben Sewell, Emily Fox, Matthew Altoft or James Cumming at either one of our Clinics.

Treatment by Scott CookNigel Smith & Karri Field will require a full consultation fee to be paid by the patient. If you have been referred to our practice by your GP under the DVA scheme you must provide a D904 referral from your Doctor and show your DVA Card in order that the necessary paperwork can be completed. Please see our Reception staff for a full copy of our policy.

SMS REMINDERS

As a courtesy we send an SMS reminder the day before your appointment. This is a reminder, not a diary note. We ask that patients take responsibility for remembering appointments. DO NOT REPLY TO OUR SMS, WE DO NOT RECEIVE REPLIES. It is far quicker and more efficient for you to ring if you wish to change your appointment.

CANCELLATION POLICY

If you are unable to keep your appointment we require 24 hours’ notice. Failure to provide notice will attract a $60 cancellation fee.
